Please complete this form about your child’s transportation to and from school for the upcoming school year (2021-2022). You should fill out the form even if your child will not need transportation (i.e., you or an approved contact plan to drop off/pick up your child at school each day).
Please note that bus changes will NOT be permitted during the school year UNLESS it is an emergency, in which case you must provide written notice to the main office of your child’s school 48 hours (two days) in advance. We understand the importance of child care and want to make arrangements that are safe and convenient for you and your child, however lack of consistency or constant bus changes are confusing and dangerous for all parties involved. If it is an emergency and you need to request changes to you child’s transportation, please notify the main office in writing as soon as possible and at least two days in advance.
