Attention, parents and students needing a physical exam for Fall Sports 2026 and/or working papers. The school’s medical provider will be at the Elementary school on August 7 from 8:30 am until 11:00 am to conduct physical exams in the Nurse’s office. Students will need a Health History Form signed by their parents with all the ‘yes’ answers fully explained.
The Health History Form is a required medical form for any student participation in interscholastic athletics.
