JHS New Entrants/7th Grade Parents/Guardians: Please read the following letter from School Nurse Stephanie Haigh!
Dear Parent/Guardian :
New York State law requires that all new entrants and 7th graders have a physical examination on file in the health office. School physicals for the 2023-2024 school year are acceptable if they were done anytime up to one year before the opening day of school September 2023. This means any physical from your physician dated after September 6, 2022 will be accepted.
Your physician’s office may fax a copy of your student’s most recent physical to 518-697-8791, Attention: Junior High School Nurse. Although it is recommended that physicals be done by your private health care provider in order to ensure consistent medical care, your child will be provided a health appraisal by the school physician if we do not receive one from you.
