New HCSD online process for working cards:
- Parents need to complete the top section of the working card application (Part I). The link is provided below. If you need to print, Staples in Hudson can print you a copy for your convenience. If you cannot get to Staples, email firstname.lastname@example.org and we will mail a form to you.
- Once Part I is complete, the child’s physician will need to provide documentation of the students latest physical and attach it to the application.

We will process working cards once a week for any applications that are received. Once the cards are completed, they will be mailed directly to the student.
